of Leaders and Innovators

 

The FHN STEM, Leadership & Academic Literacy Day Camp is a four-day immersive experience for rising high school scholars in the Atlanta Public Schools community. Through hands-on STEM exploration, leadership development, and literacy enrichment, scholars gain the skills, confidence, and network to excel in high school and beyond.

To complete enrollment, parents and guardians must download, complete, and return all required forms in person on Registration Day — Wednesday, July 22, 2026. Please review every form carefully before arriving.

All forms must be returned in person on Registration Day — Wednesday, July 22, 2026. Download, print, and complete each form below before you arrive. Scholars whose required forms are incomplete or missing may not be admitted to camp until all documents are received and verified.

 

FORM 1
Authorized Pick-Up Form

Lists individuals — other than the registered guardian — who are permitted to pick up the scholar. Scholars will only be released to verified, listed individuals. A government-issued photo ID will be required at pick-up.

FORM 2A
Medical History & Health Form

Documents existing health conditions, chronic illnesses (e.g., asthma, diabetes), immunization records, and any physical activity limitations. Please attach a physician’s note if applicable.

Allergy Information Form

Captures food, environmental, and medication allergies — including severity, symptoms, and step-by-step emergency response instructions. Required if any known allergy exists. EpiPen details are included.

Medication Authorization Form

Authorizes FHN staff to administer prescription or over-the-counter medication during camp. All medications must be in their original labeled container. Required only if the scholar requires medication during camp hours.

FORM 3A
Liability Waiver

A legally binding agreement in which the parent or guardian acknowledges the inherent risks of camp activities and releases Five Horsemen Nonprofit, Inc. from liability. Governed by Georgia state law.

Medical Treatment Consent

Grants FHN staff permission to provide first aid and seek emergency medical treatment — including hospitalization — if a parent or guardian cannot be reached during a medical emergency.

Photo & Media Release

Grants or denies FHN permission to use photos, videos, and recordings of the scholar for organizational materials, including social media, press releases, and grant reporting. Guardian may specify restrictions.

Camper Code of Conduct

Outlines behavioral expectations, STEM lab safety rules (including PPE requirements), consequences for violations, and a Scholar Pledge. Must be signed by both the scholar and a parent or guardian.

Registration Day Checklist

What to Bring on July 22

 

Arrive on Registration Day — Wednesday, July 22, 2026 — with your complete packet ready to submit. The items below are required for every scholar’s enrollment to be processed.

 
All Completed & Signed Forms
Forms 1, 2A, 3A, 3B, 3C, and 4 are required for every scholar.
 
Forms 2B & 2C (If Applicable)
Required for any scholar with allergies or who needs medication administered.
 
Parent / Guardian Photo ID
A valid, government-issued ID for the registering adult.
 
Health Insurance Card
A copy or photo of the scholar’s current insurance card is accepted.
 
Immunization Records (if not listed on Form 2A)
Official school or physician records are accepted.
 
Medications in Original Containers
Only if Form 2C was completed. No unlabeled containers will be accepted.
 
Physician’s Note (if applicable)
Required for scholars with physical activity restrictions noted on Form 2A.
 
Custody / Legal Orders (if applicable)
Attach a copy to Form 1 if any pick-up restrictions are in place.
